Step 2: Comprehensive Assessment (Within 1 Week) 

What Happens: 

  • Detailed assessment of your needs, goals, and circumstances 
  • Review of existing reports or NDIS plans 
  • Development of personalised support recommendations 

What You’ll Need: 

  • Medical reports and diagnosis information 
  • Current NDIS plan (if you have one) 
  • Information about your living situation and family dynamics 

Duration: 1-2 hours (phone, video call, or in person)
Outcome: Personalised support plan recommendations 

 

Step 3: Service Setup (1-2 Weeks) 

If you have NDIS funding: 

  • Service agreements and introduction to your support team 
  • Scheduling of regular supports and emergency protocols 

If you need NDIS support: 

  • Assistance with NDIS application and gathering evidence 
  • Connection to free community programmes while you wait 

Outcome: Clear service schedule and support team in place 

 

Step 4: Ongoing Support and Review 

  • Specialist staff providing agreed services 
  • Regular communication and coordination with healthcare providers 
  • Monthly progress reviews and plan adjustments
